Our Mission

To deliver comprehensive high quality patient-centered health and support services in the Chautauqua region

Our Vision

Our core principles are access, treatment, education, and prevention delivered by friendly and professional clinic and administrative teams.

Our Commitment

No one will be denied access to our services due to inability to pay. We offer a discounted/sliding fee schedule based on family size and income.

The Chautauqua Center is a Federally Qualified Health Center (FQHC). An FQHC is a community-based provider that receives federal funds to provide healthcare in underserved areas. FQHCs must meet a stringent set of requirements, including providing care on a sliding fee scale and operating under a governing board that includes patients.

Since opening our first site in 2013, The Chautauqua Center has been recognized as a health leader in the region. In 2017, we proudly achieved the designation of a Patient Centered Medical Home (PCMH), reflecting an excellent track record of providing advanced primary care by taking the extra step for patients and those in the community.

In 2020 we received the Economic Development Award from the Chautauqua County Chamber of Commerce, an honor reserved for an organization that has contributed most to the economic development, quality of life, and stability of our region.

Most recently, the Health Resources & Service Administration (HRSA) recognized The Chautauqua Center in three categories of achievement: Health Disparities Reducer, Access Enhancer, and Advancing Health Information Technology (HIT) For Quality.
